Job summary

Vitality Hospice & Palliative Care is seeking a Bereavement Coordinator for a hybrid, full-time position. You will provide emotional support, grief counseling, and resources to families and caregivers as they navigate loss.

The role combines remote administrative work with in-person visits, memorial services, and grief programs, with local travel as needed. You will collaborate with an interdisciplinary hospice team to deliver compassionate bereavement care.

Qualifications

• Bachelor’s degree in Social Work, Counseling, Theology, or related field, or specialized certification in bereavement counseling. Master’s degree preferred.
• Experience providing grief counseling or bereavement support with patients and families.
• Hospice or palliative care experience preferred.
• Strong understanding of grief, loss and bereavement support.
• Ability to recognize complicated grief and know when to refer for mental health support.
• Excellent communication, interpersonal and organizational skills.
• Knowledge of community resources and referral services; strong documentation and time-management skills.
• Valid driver’s license, reliable transportation, and ability to travel locally.

Responsibilities

• Provide pre-bereavement and grief counseling to patients, families, and caregivers.
• Assess emotional and bereavement needs; develop individualized support plans.
• Provide ongoing bereavement support for at least 13 months after a patient's passing.
• Coordinate and facilitate counseling, grief support groups, and programs.
• Plan memorial services and remembrance events.
• Identify at-risk individuals and refer to community resources and professionals.
• Develop and maintain bereavement records and documentation.
• Collaborate with nurses, social workers, chaplains, volunteers, and other hospice team members.
• Assist with training and coordinating bereavement volunteers.
• Provide grief education and staff support when needed.
• Ensure compliance with hospice regulations and policies.
• Participate in team meetings and on-call coverage.
