• A master’s degree in social work from an accredited college/university with a supervised field placement providing case management and/or counseling services to individuals and families.

Responsibilities

• Complete a functional assessment, develop and implement a crisis plan and individual, family, community support plan. Utilize person centered practices.
• Assess, reassess, plan, refer, link and evaluate the delivery, appropriateness and effectiveness of services over time.
• Do minimally one face to face visit per month as identified in statute with each identified person, telephone contact as needed.
