• Provide case management and care coordination with crisis and support plans.
• Perform face-to-face visits monthly and maintain ongoing contact as needed.
• Coordinate with collateral providers to ensure continuity of care.
• Prepare court reports and pre-petition screening; assist with civil commitments.
• Maintain accurate case records per state, federal, and county regulations.
