Job summary

Minnesota Merit System seeks a Behavioral Health Social Worker/Case Manager to provide case management, care coordination, and employment support for individuals meeting Rule 79 eligibility. The role integrates mental health, child welfare, addiction, and disability services with a focus on coordinated care.

Maintain case records, collaborate with partners, and advocate for services while ensuring data practices compliance.

Qualifications

• Bachelor’s degree with field placement providing case management services.
• Master’s or bachelor’s degree in related field with 1–3 years human services experience.
• Master’s in social work with supervised field placement in case management/counseling.
• Minimum 15 hours per year of continuing education or in-service training.
• Mental Health Professional licensure not required.

Responsibilities

• Complete functional assessment and crisis plan with person-centered practices.
• Assess, reassess, plan, refer, link and evaluate services over time.
• Conduct at least one face-to-face visit per month and telephone contact as needed.
• Collaborate with collateral providers to support the plan.
• Assist individuals and families in obtaining services through agency coordination.
• Maintain accurate and current case records per regulations.
• Provide outreach for diagnostic assessments when needed.
• Coordinate foster care or residential treatment and related paperwork.
