Job summary

Centene in the San Antonio area is seeking a Behavioral Health Care Manager to assess and coordinate holistic care management for members with significant mental health needs. The role is hybrid, with in-person assessments and regular home visits to evaluate needs and coordinate services.

You will educate members and families on available services, collaborate with physicians and care facilities, and ensure documentation aligns with clinical guidelines.

Qualifications

• Master's degree in Mental Health or Social Work or Nursing is required.
• Licensure as LCSW, LMFT, LMHC, LPC or RN with Behavioral Health experience is required.
• 2–4 years of related experience in behavioral health settings is required.

Responsibilities

• Evaluate the needs of the most complex and high risk members with behavioral health needs and develop a plan of care.
• Act as liaison and advocate between member/family, physician, and facilities/agencies.
• Provide education to members and families on services, benefits, and treatment options.
• Perform frequent home visits and/or site visits (monthly or more) to assess needs and coordinate resources.
• Coordinate community resources and service authorization for services (meals, housing, transportation, etc.).
• Maintain accurate documentation in the electronic care management system and ensure compliance with guidelines.
