• Evaluates member service needs and develops or contributes to care/service plans.
• Educates members, families and caregivers on services and benefits available.
• Performs frequent home and/or site visits (monthly or more).
• Provides education to long-term care members and families on preventive care, treatment options and benefits.

Kenntnisse

Behavioral health experience
