Centene Corp. seeks a hybrid Behavioral Health Care Manager to support Pearland, TX area members.

This role focuses on holistic care management for individuals with significant mental/behavioral health needs, including home visits and education for members, families, and caregivers. The incumbent will travel up to 75% for member assessments, coordinate with providers, and ensure access to services while maintaining compliance with clinical guidelines.

Formación

• Requires a master’s degree in Mental Health Counseling or Social Work.
• 2–4 years of related experience in care management.
• An unrestricted behavioral health license (e.g., LCSW/LMFT/LMHC/LPC) is required in the applicable state.
• Bilingual English/Spanish skills are a plus but not required.

Responsabilidades

• Evaluate needs of complex and high‑risk members and develop plans of care.
• Act as liaison between member/family, physician, and facilities.
• Provide education to members and families on services and benefits.
• Coordinate service authorizations and access to community resources.
• Maintain accurate documentation in the electronic care management system and ensure compliance.
