Job summary

Commonwealth of Massachusetts in Holyoke is seeking a Human Services Coordinator I to serve individuals and families, coordinate ISP development, and ensure timely services.

Qualifications

• At least three (3) years of full-time or equivalent part-time professional experience in human services work or social work.
• Bachelor's degree or higher may substitute for two years of required experience.

Responsibilities

• Coordinate and facilitate the development of Individual Service Plans (ISP) for individuals on the caseload.
• Monitor and coordinate the DDS-funded residential, day and support services.
• Advocate for individuals' human, civil and legal rights and maintain records.
• Maintain regular contact with individuals, families, providers, and community partners.
• Travel to the Holyoke/Chicopee area as needed.
