Job summary

The Commonwealth of Massachusetts seeks a Human Services Coordinator I for the Department of Developmental Services in Waltham. Qualifications

• At least three (3) years of full-time or equivalent in human services work or social work.

Responsibilities

• Coordinates and facilitates development of Individual Service Plans (ISPs) for individuals on assigned caseload.
• Monitors implementation of DDS-funded residential, day, and support services.
• Arranges and coordinates DDS-funded and generic supports to meet individualized needs.
• Maintains communication with individuals, families, guardians, provider agencies and community resources.
• Specializes as ASD SC, CSC, Adult SC, Transition SC or Self-Direction SC with duties tailored to role.
