The purpose of this position is to provide support to the professional counseling staff and provide service coordination for a caseload consisting primarily of individuals with deaf and hard of hearing disabilities in Burke County and the 16 surrounding counties in Western NC. Under the supervision of the Rehabilitation Counselor Supervisor and the Facility Director, the Human Service Coordinator:
Job responsibilities include:- Provide support to the designated counselor by completing intakes.
- Obtaining medical records for purposes of determining eligibility.
- Work with consumer in developing a plan of services, and coordinating planned services.
- Developing job resources, counseling and purchase needed services,
- Arranging for appropriate evaluations and other services indicated on the Individualized Plan for Employment.
- Responsible for documenting service delivery and case progress in the case management system according to VR policy.

- Knowledge of casework management and processes.
- Knowledge of analyzing medical, psychological, or vocational data for service delivery.
- Demonstrated experience working with the public and in groups.
Management Preferences:- Knowledge and experience working with persons with disabilities.
- Knowledge of working with community groups and agencies
