Catholic Charities Brooklyn and Queens is seeking a Housing Wellness Coordinator to join its Supportive Housing Programs for seniors. The role entails delivering on-site wellness services, conducting assessments, and providing counseling to residents to help them age in place.

Under supervision of a licensed clinician, you will develop and implement wellness activities, coordinate care with community partners, and ensure compliance with confidentiality and HIPAA regulations.

Qualifications

• Bachelor's degree required in human services or related field.
• Two years’ experience in housing, mental health, or elderly populations.
• Excellent organizational, verbal, and written communication skills.
• Bi-lingual Chinese/Korean/Spanish/English required.

Responsabilités

• Conduct and prepare assessments, case logs, and care plans for tenants.
• Establish and maintain contacts with families, social service agencies, and health facilities.
• Develop and implement service plans based on residents' needs.
• Provide counseling, behavioral interventions, and crisis management services.
• Coordinate care with building management and staff; respond to emergencies on site.

Formation
