Job summary

Catholic Charities Brooklyn and Queens, Inc. seeks an OMH-Resident Wellness Coordinator to provide rehabilitation and recovery services to residents in NYSOMH-funded programs in Brooklyn, Queens, and Jamaica.

You will deliver counseling, advocacy and case management, supporting residents to achieve service plan goals with respect and dignity. The role emphasizes integrated care, holistic assessment, and connections to internal and external services within the agency framework.

Qualifications

• Bachelor’s degree in social work, psychology or a related health/human services field required.
• Excellent clinical skills with at least two years’ relevant mental health, substance abuse or human service experience.
• Excellent organizational, verbal, & written communication skills.
• Excellent computer skills, including experience with all Microsoft Office software packages, local area networks, and the Internet.

Responsibilities

• Provide counseling, behavioral interventions, restorative services, assistance in negotiating community activities and crisis management services for OMH Residential clients.
• Carry out all duties in accordance with NYSOMH regulations.
• Meet with residents a minimum of twice a week for OMH Community Living programs and a minimum of one time per month for OMH Supported Housing Program to discuss progress.
• Prepare contact notes, incident reports and other documentation as needed in accordance with NYSOMH regulations.
• Develop wellness goals and activities with input of residents and staff.
• Make referrals to appropriate community service providers.
• Ensure and assist clients in following program rules and guidelines.
• Accompany clients to scheduled appointments with service providers as needed.
• Participate in weekly supervision, case conferences and staff meetings.
