Catholic Charities Brooklyn and Queens is seeking an OMH-Resident Wellness Coordinator to provide rehabilitation and recovery services within NYSOMH-funded Residential Programs. You will counsel residents, advocate for services, and coordinate case management to support holistic client care.

You will conduct meetings, document progress, and develop wellness plans with residents and staff, while ensuring compliance with NYSOMH regulations and agency guidelines.

Qualifications

• Bachelor’s degree in social work, psychology or a related health/human services field.
• Excellent clinical skills with at least two years relevant mental health, substance abuse or human service experience.
• Bilingual Chinese/Korean/Spanish/English preferred.
• Regularly required to talk, hear, walk, stand, & lift up to 10 lbs.
• Able to travel throughout the five boroughs as needed.

Responsibilities

• Provide counseling, behavioral interventions, restorative services, assistance in negotiating community activities and crisis management services for OMH Residential clients.
• Carry out all duties in accordance with NYSOMH regulations.
• Meet with residents a minimum of twice a week for OMH Community Living programs and a minimum of one time per month for OMH Supported Housing Program to discuss progress and document goals.
• Prepare contact notes, incident reports and other documentation per NYSOMH regulations.
• Develop wellness goals and activities with input from residents and staff.
• Make referrals to appropriate community service providers.
• Ensure and assist clients in following program rules and guidelines.
• Accompany clients to scheduled appointments with service providers as needed.
• Participate in weekly supervision, case conferences and staff meetings.
• Report issues to Behavioral Health Services Administration and/or Agency Administration as needed.
• Assess the needs of the whole consumer/family including basic needs such as food, clothing and shelter.
• Follow up on referrals and ensure continuity of care.
• Participate in cross-program service reviews and medication monitoring.
• Assist with Crisis Intervention as needed.
• Assist and instruct residents in Activities of Daily Living.
