BronxWorks is seeking a dedicated Case Manager to join Park Haven, a supportive housing program in the South Bronx. You will manage a caseload, conduct home visits, and document progress toward treatment goals, working within a Housing First model to connect clients with essential services.
The role requires strong writing and communication skills, a Bachelor's in social work, and knowledge of city resources. Schedule is Mon-Fri 9am-5pm with on-site responsibilities.

BronxWorks is seeking one Case Manager to join a diverse and enthusiastic team at Park Haven, a supportive housing program located in South Bronx. Park Haven provides on-site case management services and permanent housing to 50 formerly homeless households, consisting of both single individuals and families with children. The ideal candidate will understand the Housing First model and will work comprehensively with their caseload to work on various tasks that include, but are not limited to, conducting home visits, escorts in the field to various appointments, and completing all necessary documentation in a timely manner. Work schedule is Mon-Fri 9am-5pm.
