DESC is seeking a Housing SUD Counselor / Case Manager for the Keys to Home program in Seattle. You will develop Residential Services Plans, coordinate services with outside providers, and monitor progress with residents, documenting engagement and outcomes.

The role emphasizes collaboration with DESC staff, providers, and residents, including crisis intervention, group facilitation, and organizing activities. Union representation by SEIU Healthcare 1199NW is provided.

Qualificações

• Licensed by the State of Washington as a Substance Use Disorder Professional (SUDP).
• Experience with homeless adults with chronic alcoholism, mental illness, and drug use.
• Knowledge of local homeless, mental health, and chemical dependency service systems.
• Ability to communicate and work with individuals from diverse backgrounds.
• Ability to work with clients displaying a wide range of behavior.
• Ability to perform major duties with or without accommodations.

Responsabilidades

• Develop Residential Services Plans (RSP) with residents and outside providers.
• Coordinate case management elements with outside providers and directly provide others.
• Maintain regular contact with residents and providers to monitor progress and services.
• Record progress notes in RSP files and daily logs.
• Facilitate resident meetings and organize on-site activities.
• Assist in building rule enforcement and other property management tasks.
