Job summary

Downtown Emergency Service Center (DESC) seeks a dedicated Case Manager to support adults with severe mental illness through coordinated services, housing linkage, and crisis intervention. You will work within the SAGE program to connect clients with housing resources and community supports.

Qualified candidates will have a social work background; a licensed credential is preferred. This role emphasizes collaboration with multiple teams and a commitment to recovery-focused care in a unionized

Qualifications

• Bachelor’s degree in social work, psychology, or related behavioral science.
• Experience in housing services or behavioral health setting is preferred.
• Bilingual Spanish/English is a plus.

Responsibilities

• Participate in multi-disciplinary team providing ongoing case management for adults with severe mental illness.
• Collaborate with housing teams to develop client care and housing retention plans.
• Provide outreach and engagement to assigned clients; build working relationships.
