Job summary

DESC is seeking a Clinical Support Specialist for day shifts in Seattle, WA. The role collaborates with an integrated care team to provide case management and mental health services to tenants, coordinating with external providers and DESC staff to support housing stability.

The position requires licensure in Washington or the ability to obtain soon after hire, plus experience working with homeless adults with complex needs. Union representation is noted; union benefits apply.

Qualifications

• Ability to perform all major and assigned duties and responsibilities with or without accommodations.
• Current AAC license in Washington State, or equivalent, or ability to obtain soon after hire.
• Clinical experience with homeless adults affected by chronic conditions and mental health challenges.
• Knowledge of local homeless, mental health, and chemical dependency services.

Responsibilities

• Facilitate engagement of tenants with culturally relevant strategies and publicize service availability.
• Develop and routinely update an Integrated Service Plan (ISP) with tenants and staff.
• Coordinate case management elements with DESC or outside providers.
• Support tenants in maintaining safe unit conditions and coordinating caregiver services.
• Provide and document case management services not covered by other providers.
• Maintain regular contact with tenants and DESC staff to monitor ISP goals.
