Job summary

The Economic Opportunity Council of Suffolk Inc in New York is seeking a Housing Specialist to provide housing stabilization and tenancy sustaining services to Medicaid members. You will assess housing needs, perform case management, and coordinate access to enhanced housing services.

Responsibilities include intake, eligibility verification, coordination with landlords and utility providers, housing habitability inspections, and documenting outcomes in Unite Us.

Qualifications

• Must have strong case management and housing services experience.
• Proficiency with Unite Us for referrals and documentation.
• Ability to manage an active caseload (~40 members) and meet deadlines.
• Valid New York driver's license and insured vehicle.

Responsibilities

• Provide housing stabilization and tenancy sustaining services to Medicaid members.
• Assess housing needs and coordinate Enhanced Housing Services access.
• Accept, track, and manage referrals for Tenancy Sustaining Services.
• Document assessments and outcomes in Unite Us accurately.
• Maintain monthly contact with active members to monitor progress.
• Collaborate with landlords, utilities, and community partners.
• Conduct housing habitability inspections and ensure standards.
• Attend trainings and participate in program monitoring.
