Qualifications

• Associate’s degree preferred in social work, human services, or related field.
• Experience with homelessness, housing instability, or related crises preferred.
• Familiarity with Permanent Supportive Housing and landlord engagement desirable.
• HMIS or comparable database experience desirable.

Responsibilities

• Develop and maintain housing-focused service plans to support stability.
• Assist with tenancy, lease requirements, landlord communication and recertifications.
• Coordinate rental assistance and housing-related financial supports per guidelines.
• Collaborate with landlords and partners to support tenancies.
• Maintain participant records and manage data in HMIS and agency systems.
• Participate in supervision, training, and team meetings.
• Ensure confidentiality and compliance with policies and funder regulations.
