Job summary

Qualifications
- Bachelor's or Master's degree in social work or related field.
- At least three years of relevant experience.
- Experience with housing services or HUD programs is a plus.
Responsibilities
- Screen housing services referrals for eligibility.
- Coordinate continuity of services with partner agencies.
- Attend required meetings and trainings.
- Meet with clients at least monthly.
- Provide internal referrals for employment and financial coaching.
- Develop and revise goal plans and budgets for housing stability.
- Maintain accurate client notes and HMIS data entry.
- Work with landlords to secure safe, sanitary housing.
- Assist in moving clients to new rental units.
- Maintain client files and prepare program reports.
- Coordinate efficient service delivery with staff to avoid gaps.
