Job summary

Jewish Family Service of Los Angeles is seeking a Social Worker I within the Homeless Prevention team. You will deliver care coordination and housing-focused case management for older adults and adults with complex needs, including assessing and enrolling clients, creating housing plans, and coordinating benefits.

The role requires a BA in Social Work or related field, strong communication, and the ability to work independently and as part of a team, including in home or community settings.

Qualifications

• BA degree in Social Work or a related field.
• Experience with case management, care coordination, housing stabilization for older adults preferred.
• Knowledge of community resources, public benefits, eviction prevention, aging/disability services.
• Excellent verbal and written communication; experience with telehealth platforms.

Responsibilities

• Provide care coordination and housing-focused case management to support safety, housing stability, and independence.
• Complete intake, enrollment, assessments, and required documentation.
• Develop individualized service, care, and housing plans with clients.
