Job summary

Jewish Family Service LA in North Hollywood, CA, is seeking a Social Worker I to conduct in-depth intakes and assessments for frail, elderly clients, including Holocaust survivors and families. You will evaluate barriers to independent living and activate resources to keep clients safely at home.

Qualifications

  • Bachelor's degree in social work or a related field.
  • Current unrestricted CA Driver’s License and proof of auto insurance.
  • Experience and interest in providing care to older persons.
  • Care management and community-based service delivery experience.
  • Knowledge of community resources and the aging process.
  • Experience working with seniors, caregivers and mentally disabled clients.
  • Knowledge of safety issues when working with at-risk clients.
  • Experience conducting in-home visits preferred.
  • MS Office applications proficiency.

Responsibilities

  • Conducts in-depth intakes and assessments for Holocaust survivors and their families.
  • Conducts case management meetings in the home.
  • Develops comprehensive care plans including all social services.
  • Coordinates delivery of services in the care plan.
  • Responds to intake calls and follows up on client situations.
  • Reviews and authorizes services monthly.
  • Provides crisis intervention and outreach as required.
  • Prepares quarterly reports for funding sources.
