Jewish Family Service LA (JFSA) is seeking a dedicated Social Worker I to join our Homeless Prevention team in Los Angeles. This position will focus on providing care coordination and housing-focused case management for older adults and dependent adults who are experiencing homelessness or at risk of homelessness.

As part of a larger Homeless Prevention team with multiple programs. Adult Protective Services (APS) Integrated Care Services (ICS)/Home Safe Social Worker I is responsible for providing care coordination, housing-focused case management, and supportive services to older adults age 60+ and dependent adults age 18-59 referred by APS who are experiencing homelessness, at risk of homelessness, or in need of short-term support to maintain safety, housing stability, and independence. The Social Worker I will provide services that include but are not limited to enrollment; assessment; home visits; information and referrals; housing stabilization; landlord mediation; eviction prevention support; assistance with budgeting and/or money management; assistance with accessing government benefits; developing a Housing and Services Plan; and coordinating client relief and housing-related financial assistance to help clients maintain safety and long-term housing stability

Qualifications

• BA degree in Social Work or a related social services or health-related field.
• Prior experience providing case management, care coordination, housing stabilization, and community-based service delivery experience with the older adult population preferred.
• Knowledge of community resources, housing resources, public benefits, eviction prevention resources, aging/disability services, and landlord/tenant rights preferred.
• Excellent verbal and written communication skills and ability to utilize telehealth platforms.
• Experience conducting field-based home visits and community-based service delivery strongly preferred.
• Knowledge of HMIS or other record database management systems preferred.
• Must be computer literate in MS Office applications, including Word, Excel, Outlook, SharePoint, Teams, and ability to learn new systems.
• Ability to work with clients who may have complex needs, including cognitive impairment, behavioral health needs, medical needs, mobility limitations, hoarding concerns, housing instability, or limited natural supports.
• Ability to work in a culturally sensitive, non-judgmental, and client-centered manner with a multicultural population who may have complex needs; work independently and as part of a team while maintaining professional boundaries.
• Must follow documentation, confidentiality, HIPAA, APS, County, funder, and JFSLA requirements.
• Demonstrate strong judgment and ability to budget when coordinating client services and financial assistance.
• Must pass a criminal background check, including Live Scan fingerprinting, prior to beginning employment.
• Current unrestricted California Driver’s License and proof of auto insurance required.

Responsibilities

• Provide care coordination and housing-focused case management services to support clients’ safety, housing stability, and independence.
• Complete intake, enrollment, assessments, and obtain required client documentation in accordance with program scope of work and agency requirements.
• Develop and maintain individualized service, care, and housing plans with clients, including goals related to housing stability, safety, benefits, budgeting, health, social connection, and community support.
• Conduct case management meetings with clients in the home, community, office, shelter, or other appropriate locations based on client need and program requirements. Work schedule may need to be adjusted to accommodate clients.
• Provide housing stabilization services, including support with housing applications, landlord mediation, eviction prevention, advocacy, housing inspections, repairs, and referrals to community resources.
• Assist clients with budgeting, money management, rent payment planning, and connection to government benefits resources.
• Coordinate client relief financial assistance and ensure all required documentation, approvals, receipts, invoices, and follow-up are completed accurately in a timely manner.
• Provide referrals to County and community resources, including DPSS, benefits programs, legal services, mental health services, medical care, transportation, senior centers, caregiver support, food resources, social isolation supports, and other appropriate services.
• Assist clients with obtaining identification documents and other documents needed for housing, benefits, and service access.
• Maintain current and accurate documentation in required databases, including case notes, assessments, service plans, required reports, tracking logs, financial assistance documentation, referrals, and outcomes.
• Maintain professional boundaries and use respectful, client-centered communication with clients, staff, vendors, and community partners.
• Participate in team meetings, case conferences, program meetings, supervision, training, and funder meetings as assigned.
• Other related duties as assigned.
