Job summary

Abode Services is seeking a Service Coordinator for Alameda County to provide housing services, counseling, and referrals to help participants achieve permanent housing stability. The role involves coordinating services, maintaining client files, and building linkages to community resources.

A Bachelor's degree and 2 years of case management experience are required, along with a valid California driver’s license and reliable vehicle.

Qualifications

• Bachelor's degree required or equivalent field experience.
• 2 years of case management experience with homeless or low-income individuals or families.
• Driver's license and access to a personal vehicle with current auto insurance; DMV record clean.

Responsibilities

• Provide comprehensive housing services to participants, including landlord coordination.
• Coordinate with partners to refer and provide services to participants.
• Develop and maintain individualized service plans and needs assessments for households.
• Meet with households in person, in the community or at home as needed.
• Attend program meetings with internal and external partners to coordinate services.
