Job summary

Advantage Behavioral Health Systems in Georgia is hiring a paraprofessional who will deliver social services and case management to individuals and families facing homelessness or housing insecurity. The role focuses on ESG Rapid Rehousing and ESG Homelessness Prevention, including enrollment, treatment planning, housing navigation, and service linkage.

You will collect data per grant guidelines, coordinate with clinics and community partners, conduct home visits, and provide crisis intervention

Qualifications

• Bachelor's degree in Social Services or 4+ years related experience required.
• Experience with homeless populations and case management preferred.
• Knowledge of HMIS and Coordinated Entry processes.
• Strong communication and relationship-building skills.
• Ability to work in a fast-paced environment and multi-task.

Responsibilities

• Enroll clients in ESG Rapid Rehousing and ESG Homelessness Prevention programs.
• Perform program enrollment, intake, and treatment planning.
• Housing navigation and linkage to resources.
• Data collection and grant reporting compliance.
• Coordinate with county clinics and community partners.
• Data entry into HMIS and related systems; monitor progress toward goals.
• Conduct home visits and provide community-based case management.
• Provide crisis intervention and discharge planning follow-ups.
