Qualifications

Job description

Position Summary:This position provides a broad range of paraprofessional social services and case management duties to individuals and families experiencing homelessness or housing insecurity. The focus of this position is to provide services to clients enrolled in the ESG Rapid Rehousing and ESG Homelessness Prevention programs. Services include program enrollment, treatment planning, housing navigation, direct support, screening for benefits, case management, service linkage, advocacy, problem solving, housing navigation, discharge planning, follow up, and other activities to improve housing stability. Services are provided on site and in the community. This position is responsible for data collection and reporting requirements associated with grant provider guidelines including the ESG program and any other relevant reporting requirements.Job Responsibilities and Performance Standards:Service CoordinationCompletes program enrollment, client folders, treatment planning, and other intake activitiesInitiates and maintains relationships with Advantage County Clinics to promote access to services for those living within targeted countiesInitiates and maintains relationships with identified community partners to promote effective service coordination and community response to the needs of those within the program’s target populationMaintains proficiency in eligibility requirements and intake/screening and assessment processes within county clinics and community partners to support successful linkage to available resourcesPrepares payment order requests and compiles necessary documentation for rent, utility, and other eligible program expenditureMaintains relationships with local property managers and affordable housing providerData Collection & ReportingCollects and maintains required data and supporting documentation in order to maintain program adherence to grant reporting guidelinesWorks with Team Lead to ensure data is comprehensive, complete, and accurate for creation of program performance reportActively facilitates client service enrollment process to support Advantage billing processes and requirements, as applicableCoordinates entry of data into local HMIS systemMonitors and documents progress towards treatment goals within HMIS and Health Record System, as appropriateConducts follow-up activities to track outcomes of services and referrals provideDirect ServicesAssists homeless individuals and families in locating and utilizing community resources including legal, medical, financial assistance, and other referral servicesAssists clients with housing navigation and securing permanent housing opportunitiesConducts routine home visits and community-based case management servicesProvides collaborative problem solving support to emergent needs and situations impacting clients and property managersWorks closely with clients to assess needs, establish goals, create action plans, complete applications for appropriate services, and conduct follow upsOngoing review of client progress and service note updates to document client progress toward stabilitySupports clients within the community or clinic setting and provides skill building services to assist with financial and social skillsPrepares intake reports and case history recordsProvides crisis intervention to clients with emergency situations and service linkage to appropriate emergency servicesSample Technical Competencies:Ability to build relationships among members of the target population, local business community, and representatives of local community service organizationsProvide clinically sound treatment services based on client needs and treatment plansView, inspect, review, and analyze documents, spreadsheets, policies, etc.Respond to a range of clients’ needs using differential diagnoses and treatment approachesProvide appropriate crisis intervention servicesDemonstrate understanding of sound ethical practice in conjunction with the established and defined ethics of one’s training and disciplineMust effectively demonstrate appropriate boundary setting with potential clients to support ethical service delivery and promote self empowerment and determinationEffectively organize time and responsibilitiesAbility to work in a fast paced environment; and ability to multi task and handle varying tasks simultaneously to meet deadlinesWork as part of a team, with other program staff.Minimum Qualifications:Bachelor’s level degree in Social Services, or 4 years of related experience and/or training of mental health, working with homeless clients and their families, case management, and/or community/in home based servicesPreferred Qualifications:Bachelor’s Degree from an accredited educational institution in Psychology, Social Work, or other relevant field AND 2 years of related experience and/or training of mental health, working with homeless clients and their families, case management, or community/in home based services.Experience with Individual Service Plans/Treatment Plans, familiarity with clinical terminology, and demonstrated program management experience and capabilities.Knowledge of and/or proficiency with Client Track Homeless Management Information System and Coordinated Entry process.Demonstrated passion working with unhoused individuals
