Job summary

Advantage Behavioral Health Systems in Athens, GA is seeking a paraprofessional social services and case management professional to support individuals and families experiencing homelessness or housing insecurity. Qualifications

• Minimum qualifications include a Bachelor’s level degree in Social Services, or 4 years of related experience in mental health or homelessness services.
• Experience with Individual Service Plans/Treatment Plans and clinical terminology preferred.
• Familiarity with HMIS and Coordinated Entry processes.

Responsibilities

• Complete program enrollment, intake, and treatment planning.
• Develop and maintain relationships with clinics and community partners to promote access to services.
• Ensure data collection and reporting for grant requirements including HMIS and program reporting.
• Coordinate data entry into HMIS system and monitor progress toward treatment goals.
• Provide direct services including housing navigation, referrals, and case management support.
• Prepare intake reports and maintain client case histories.
• Offer crisis intervention and link clients to emergency services when needed.
