Qualifications

• Lived experience recovering from mental illness or as a caregiver.
• Valid driver’s license and reliable transportation.
• Medi-Cal Peer Support Specialist Certificate preferred; AMIH can fund.
• Strong computer proficiency with common office tools and online platforms.

Responsibilities

• Maintain an active caseload of participants requiring intensive, community-based support.
• Build trusting relationships by applying recovery principles such as self-determination, empowerment, and resilience.
• Collaborate with clinical staff and multidisciplinary team members to support coordinated care.
• Provide outreach and engagement services in the community and various settings.
• Assist participants in accessing housing resources, benefits, healthcare services, and other supports.
• Help participants develop individualized recovery and wellness plans.
• Provide practical support such as transportation and accompaniment to appointments.
• Identify early warning signs of relapse and develop stability strategies.
• Facilitate peer support groups and wellness programming when appropriate.
• Document services in accordance with agency and program requirements.
• Advocate for client needs within the team and link clients to resources.
• Maintain up-to-date knowledge of community resources.
• Document progress using an electronic health record system.
• Report to multiple supervisors and manage responsibilities across teams.
