Job summary

Sarah's Circle is hiring a Rapid Rehousing (RRH) Case Manager in Chicago for a full-time, non-exempt role. The position offers a hybrid schedule at the Uptown location with travel to off-site sites as needed. A vehicle is required.

Typical hours are 9:00AM-5:00PM, Monday–Friday, with occasional evenings for client meetings. LCSW certification supervision may be available. Ideal candidates have experience with homelessness, mental health, and trauma-informed care.

Qualifications

  • Bachelor’s degree in psychology, social work, or related field preferred.
  • Bilingual English/Spanish preferred for translation tasks.
  • Experience with homelessness, mental illness, and/or trauma preferred.

Responsibilities

  • Provide intensive case management for RRH clients to promote housing stability.
  • Assess eligibility and develop individualized housing plans with clients.
  • Coordinate services with external agencies and housing providers.
  • Advocate for benefits and housing needs, including evictions and negotiations.
  • Assist with documentation, data entry in HMIS, and required reporting.
  • Travel to off-site locations and client homes as needed with a reliable vehicle.
  • Maintain client contacts and ensure safety, trauma-informed care, and confidentiality.
