Job summary

A Safe Place in Lake Forest, IL seeks a Transitional Housing Case Manager to secure and place survivors in transitional housing and rapid rehousing programs. You will coordinate with landlords, complete VISPDAT assessments, and place clients in housing while supporting their empowerment and independence.

The role requires flexible scheduling, crisis response, and strong communication. You will collaborate with ASP staff and community partners to access services for survivors and children.

Qualifications

• Bachelor in Social Work, Counseling, Education, or closely related field.
• Flexible schedule to meet needs of clients and crisis response.
• Bilingual preferred.
• Ability to motivate clients toward action and independence.
• Excellent written and oral communication; record documentation and training materials.

Responsibilities

• Develop partnerships with landlords and educate on policies to keep families safe.
• Provide information on domestic violence laws and protective orders to enhance safety.
• Manage applications, updates, and re-certifications to maintain stable housing.
• Oversee temporary housing for two families in ASP-owned units.
• Conduct outreach to community organizations and landlord associations to identify housing opportunities.
• Coordinate intake assessments and eligibility evaluations for services.
• Create service plans and coordinate in-house employment, education, and counseling services.
• Ensure accurate, timely documentation in client records.
