Job summary

The Housing Stabilization Case Manager at Southeastern Vermont Community Action works with individuals and families experiencing homelessness or housing instability, helping them move toward safe, stable housing.

This role is grounded in the belief that people in crisis deserve to be treated with dignity, patience, and genuine respect — and that lasting change happens when clients are seen as capable partners in their own stability.

Qualifications

• High school diploma or equivalent; degree preferred in social services.
• Experience in human services with housing instability populations.
• Strong relationship-building and assessment skills.
• Ability to document accurately and meet grant reporting requirements.
• Valid driver’s license and reliable transportation.

Responsibilities

• Conduct intake and comprehensive needs assessments using approved tools.
• Develop individualized housing plans with clients centered on goals.
• Assist clients in applying for subsidized and market-rate housing and advocate with landlords as needed.
• Connect clients with benefits and employment resources and coordinate with partners.
• Maintain HMIS records and required documentation; ensure data accuracy.
• Collaborate with the Family Services team to ensure coordinated, client-centered services.
