Job summary

Volunteers of America Colorado is seeking a Housing Case Manager to provide direct services and housing support at the Lakewood Navigation Center. The role uses best practices such as Housing Focused care, Trauma-Informed Care, and Crisis De-escalation while guiding participants toward housing goals.

Responsibilities include assessments, eligibility determinations, and coordination with landlords and service providers. Strong ethics and accurate data entry are essential for grant reporting.

Qualifications

• Bachelor’s degree in human services, social work, or a closely related field is required.
• Good organizational and planning skills; works well as part of a team.
• Ability to uphold Social Work Code of Ethics and confidentiality.

Responsibilities

• Conduct needs-based assessments and provide individualized case management to support shelter stability and housing goals.
• Assist participants in securing and maintaining housing, including rent calculations and landlord mediation.
• Collaborate with community providers and multidisciplinary teams to deliver tailored supportive services.
• Maintain records in HMIS, Service Point, and grant-relevant documentation.
• Develop and monitor housing-focused services to promote participant stability.
