Job summary

The Coordinating Center in Montgomery County seeks a Homeless Support Services Coordinator to provide person-centered care coordination for the Housing Initiative Program. You will engage program recipients, develop individualized recovery-based housing plans, and collaborate with CES, HIP Housing Locator, and health staff to ensure housing stability and access to benefits.

Qualifications

• A bachelor’s degree in a human services related field is required.
• Two years of case management experience is required.
• Ability to travel throughout Montgomery County and conduct home and community visits is required.

Responsibilities

• Utilize the Housing First model to engage program recipients and promote housing stability.
• Work one-on-one with individuals to provide support, engagement, and referral.
• Coordinate housing and services to maintain residential stability and connect with CES, HIP locators, and health nurses.
• Document care management activities in electronic databases, including HMIS.
• Engage in regular meetings and trainings to advance best practices in homelessness services.
