Job summary

The YWCA Greater Harrisburg is seeking a full-time Transitional / Rapid Rehousing Case Manager to help individuals and families experiencing homelessness access safe, affordable housing. You will perform intakes, develop service plans, connect clients with resources, and coordinate with landlords and partners to promote housing stability.

Qualifications

Bachelor's degree in human services or related field; two years of experience preferred.
Knowledge of housing stability, intake, and service-planning processes.
Valid driver's license and reliable vehicle.

Responsibilities

Conduct client intakes, assessments, and individualized goal planning.
Provide weekly case management and advocacy, including referrals.
Assist clients in securing housing and understanding leases.
Maintain accurate records and required reporting.
Collaborate with property management and community partners.
Participate in on-call rotations.
