Job summary

YWCA Greater Harrisburg is seeking a full-time Permanent Supportive Housing Case Manager to join our team. The role focuses on providing comprehensive case management and housing stabilization services to individuals and families experiencing homelessness through HUD-funded programs.

You will develop service plans, advocate for clients, coordinate resources, and ensure program compliance while promoting long-term housing stability and self-sufficiency.

Qualifications

• Bachelor's degree in Human Services or related field OR Associate Degree with two years' experience OR associate degree equivalent with four years' experience.

Responsibilities

• Provide case management and housing stabilization services to individuals and families experiencing homelessness.
• Assess eligibility, develop individualized service plans, and connect clients with community resources.
• Help clients secure and maintain safe, affordable housing while ensuring HUD and program compliance.
• Build partnerships with landlords and community agencies to support successful housing outcomes.
• Maintain accurate documentation, case records, and required grant reporting.
• Participate in community collaborations, the program's on-call rotation, and other duties as assigned.

Education
