Job summary

Plymouth Housing Group seeks a Case Manager to support residents on the onsite Resident Well-Being Care team. You will provide services to housing‑stability focused residents, maintain ISPs, and promote stability through trauma‑informed and harm‑reduction approaches.

This is a unionized, in‑person role at Seattle sites. You will manage a caseload, collaborate with internal departments, and coordinate with community partners to maximize access to benefits and resources, ensuring dignity and

Qualifications

• BA/BS degree in a human service field required or equivalent experience.
• Minimum 1 year of experience as a case manager in housing, behavioral health, or social services.
• Experience in Permanent Supportive Housing preferred.

Responsibilities

• Develop and maintain ISPs with residents to address housing stability and goals.
• Identify and connect residents with community resources and public benefits.
• Schedule transportation and accompany residents to appointments.
• Advocate and collaborate with providers to ensure continuity of care.
• Coordinate with Housing Operations to support stability and housing-related tasks.
• Respond to crises using trauma-informed, harm-reduction approaches.
