Qualifications

Responsibilities

• Provide on-site, in-home case management, resource referrals, and follow-up to residents, advocating for access to services.
• Maintain complete, confidential resident records and program data to meet goals and contractual requirements.
• Participate in department, organization, and community meetings and trainings, including anti-racism initiatives.
