Job summary

St. Vincent de Paul CARES in Tampa, FL is seeking a Case Manager III to provide assessment, planning and advocacy for veterans and their families within the Supportive Services for Veteran Families program.

You will conduct crisis intervention, develop Housing Stability Plans, monitor progress, and link clients to housing and community resources while maintaining thorough documentation and adhering to program rules and government regulations.

Qualifications

• Bachelor’s degree in social work or related field.
• Minimum two years’ experience serving homeless or at-risk populations in crisis.
• Proven ability to assess client needs and develop housing plans.
• Ability to comply with complex government regulations and document compliance.

Responsibilities

• Provide crisis intervention for very low-income, homeless veterans.
• Assess strengths and barriers to help develop housing goals.
• Design and execute a Housing Stability Plan for each household.
• Monitor progress and maintain accurate case documentation.
• Ensure compliance with program rules and government requirements.
• Refer clients to services and resources as needed.
• Facilitate transition to transitional or permanent housing when appropriate.
