Job summary

St. Vincent de Paul CARES is seeking a Case Manager IV in New Port Richey, FL. You will provide case management to shelter clients, help them achieve self-sufficiency and coordinate housing plans and community resources. You will support other case managers and review case handling to optimize resource use.

The role requires knowledge of harm reduction, trauma-informed care, and strong documentation practices. A Level 2 background check is required by Florida standards.

Qualifications

• Bachelor's Degree in Social Services or equivalent experience.
• Minimum 5 years in social services or related field.
• Ability to work with diverse populations and communities.

Responsibilities

• Set attainable goals and monitor client progress toward self-sufficiency.
• Design housing plans and coordinate needed supports for clients.
• Assist clients with housing searches, information, referrals and veteran services.
• Maintain accurate documentation and report progress to management.
• Promote teamwork and collaborate with agency staff and external partners.
