Job summary

The role focuses on assessing needs, developing tailored housing goals, and linking clients to community resources while maintaining accurate documentation. Qualified candidates will have a bachelor’s degree in social work or related field and at least 2 years’ experience with homeless or at-risk populations, plus strong

Qualifications

• Bachelor's degree in social work or related field.
• Minimum 2 years’ experience serving homeless or at-risk families/individuals.
• Experience with wrap-around services in CoC preferred.
• Proficient in data management and documentation.
• Able to comply with complex governmental regulations and program policies.

Responsibilities

• Screen households to determine eligibility.
• Assess individuals and families to develop housing goals and plans.
• Develop Housing Stability Plan within one week of entry.
• Monitor progress and document outcomes.
• Coordinate with the Coordinated Entry System and HMIS.
• Attend RRH provider meetings and adhere to policies and procedures.
• Provide referrals and crisis interventions as needed.
