Job summary

The Connection is hiring a Case Manager 2 in Waterbury, CT to deliver home- and community-based case management through the START program. The role emphasizes trauma-informed care and youth development, with a focus on housing stabilization and tenancy support.

Responsibilities include intake assessments, developing service plans, coordinating with landlords and service providers, and supporting youth toward self-sufficiency.

Qualifications

• Associate’s degree required.
• 2+ years of direct service and case management experience.
• Valid Connecticut Driver’s License.

Responsibilities

• Deliver comprehensive case management services in clients' homes and communities.
• Conduct intake assessments.
• Assist youth in locating available apartments for rent.
• Develop and implement individual service plans toward self-sufficiency.
• Liaise with treatment providers and community resources.
• Provide transportation to clients as needed.
• Develop money management and employment readiness skills.
• Liaise with landlords and property management staff to support tenancy.
• Data entry and documentation.
• Caseload typically 8–12 young adults.
