Job summary

The Connection Inc. in Bridgeport, CT is seeking a Case Manager 2 to deliver home- and community-based case management for homeless young adults. You will assess needs, help locate housing, and implement individual service plans to progress toward self-sufficiency.

You will liaise with landlords and providers, arrange transportation as needed, and maintain accurate documentation for a caseload of 8–12 clients, supporting stability and housing outcomes.

Qualifications

• Associate’s Degree in a relevant field is required.
• Minimum 2 years direct service/case management experience.
• Valid Connecticut Driver’s License required.
• Regular and predictable attendance is required.

Responsibilities

• Provide home- and community-based case management to homeless young adults.
• Conduct intake assessments and support housing placements.
• Develop and implement individual service plans toward self-sufficiency.
• Coordinate with treatment providers and landlords; assist with transportation.

Program: STARTPosition: Case Manager 2Location: BridgeportSalary: $22.00/hourSchedule: Full time, 40 hours, 1st shift, Monday-FridayProgram Summary:The Start Program is a nationally recognized program that works with homeless and unstably-housed youth, age 18-24, located throughout the state of CT through an innovating service delivery model that combines short-term rental assistance with intensive, community-based case management services targeted towards housing stabilization. Through a lens of trauma-informed care and positive youth development, the program partners with young adult participants to identify goals and resources that help them to exit homelessness and achieve long-term stability.Position Summary:The Case Manager 2 is responsible for providing home- and community-based case management services to homeless young adults located throughout the state.
