The Connection Inc. in Bridgeport, CT seeks a Case Manager 2 to provide home- and community-based case management to homeless young adults across the state.

You will conduct intake assessments, help clients find rental housing, and develop tailored service plans to promote self-sufficiency. Responsibilities include coordinating with treatment providers, offering transportation, building money management and employment readiness skills, and maintaining accurate documentation.

Qualifications

• Must have Associate’s Degree.
• 2+ years of direct service and case management experience.
• Valid Connecticut Driver’s License required.

Responsabilités

Program: STARTPosition: Case Manager 2Location: BridgeportSalary: $22.00/hourSchedule: Full time, 40 hours, 1st shift, Monday-FridayProgram Summary:The Start Program is a nationally recognized program that works with homeless and unstably-housed youth, age 18-24, located throughout the state of CT through an innovating service delivery model that combines short-term rental assistance with intensive, community-based case management services targeted towards housing stabilization. Through a lens of trauma-informed care and positive youth development, the program partners with young adult participants to identify goals and resources that help them to exit homelessness and achieve long-term stability.Position Summary:The Case Manager 2 is responsible for providing home- and community-based case management services to homeless young adults located throughout the state.
