The candidate will assess needs, develop care plans, provide direct client support, and link to health services while engaging with internal and external providers to ensure coordinated care and adherence to program goals.

Responsibilities

• Coordinate housing resources and navigate housing systems for clients.
• Counsel and support high-risk, vulnerable individuals including those with substance use issues.
• Assess client strengths and needs to develop care plans.
• Linkage and referral to health and social services.
