Schenectady Community Action Program Inc. seeks a full-time housing and family stabilization case manager in Schenectady, NY. The role centers on case management, assessments, goal setting, and advocacy to help families exit poverty and achieve stable housing.

Requirements include a 4-year degree in social work or related field; 2 years’ related experience (Master’s preferred). Hybrid work is possible with some remote flexibility; a NY driver’s license is required.

Qualifications

• Bachelor’s Degree in social work, human services or related field; Master’s Degree preferred.
• Two years’ experience providing related services; one year with a Master’s Degree.
• Must have a valid NY driver’s license and access to a car; travel to off-site locations.

Responsibilities

• Case management to assist overcoming barriers to housing and family stabilization using a strengths-based approach.
• Complete comprehensive assessments of household strengths and needs and set goals with participation.
• Evaluate progress regularly and adjust goals to support retention of permanent housing.
• Ensure access to SCAP services and coordinate with partners to streamline support.
• Advocate for households in a solution-focused manner and represent SCAP professionally.
• Conduct weekly home visits and interact with customers at off-site locations.
