Qualifications

• Experience in social services or case management is required.
• Bachelor’s degree or HS diploma with relevant experience as specified.
• Ability to obtain pass physical.

Responsibilities

• Arrange and attend home visits.
• Prepare transition plans and coordinate services.
• Coordinate Independent Living Services.
• Serve as liaison between Schools and Residential.
• Complete assessments such as FASP and SPR.
• Maintain case notes in the Connections database.
