Job summary

St. Catherine’s Center for Children in Albany, NY, seeks a Case Manager for Marillac Shelter. You will perform intakes, assessments, and develop service plans to help families achieve housing stability, coordinating services and transportation as needed.

The role requires strong advocacy, crisis intervention, and compliance with HIPAA and program standards. Flexible schedule Mon–Fri, 9:00am–5:00pm, with some after-hours as needed.

Qualifications

• Associate degree in a related field with two years of relevant experience; BSW or Bachelor’s Degree in a Human Services field preferred.
• Will consider previous professional and/or life experience in lieu of a college degree (High School Diploma or GED equivalent required).
• Previous experience in case planning or case management preferred.
• Previous experience working in a human services agency is a plus!

Responsibilities

• Conduct intake, assessments, and develop individualized service plans to support housing stabilization.
• Provide intensive case management to stabilize conditions contributing to homelessness and support skills development.
• Coordinate transportation for families to medical, social service, and housing appointments.
• Advocate for clients within public assistance and housing networks; navigate community services.
• Intervene in crises and facilitate mediation to maintain housing stability.
• Maintain thorough documentation and complete required quality improvement reports.
