Job summary

Care Resource Community Health Center is seeking a Housing Case Manager to assess and coordinate Housing Opportunities for Persons with AIDS (HOPWA) and other housing programs for clients. The role involves screening for HOPWA eligibility, completing program applications, coordinating referrals, and maintaining accurate documentation to support housing stability and client care.

Travel to client sites may be required to connect resources and services.

Qualifications

• 2 years: Experience with homeless/social services required.
• Knowledge of housing programs (HOPWA, Section 8, rapid rehousing) beneficial.
• Ability to input data accurately into electronic health records and related systems.

Responsibilities

• Screen and assess new clients for HOPWA services and other housing stability programs.
• Determine eligibility for HOPWA program and other housing services.
• Complete applications for HOPWA programs and other housing programs.
• Identify resources and refer applicants to appropriate housing programs.
• Maintain progress notes and records per agency policy.
• Input client information using electronic data-entry per guidelines.
• Maintain an accurate timesheet reflecting time spent on programs.
• Coordinate referrals and advocate for housing resources with internal and external partners.
• Participate in staff trainings and contribute to program reports.
