Job summary

Community of Hope is seeking a Housing Stability Case Manager for our Homelessness Prevention Program in Washington, DC. The role involves providing personalized support, connecting clients with resources, and empowering families to overcome barriers to stability.

The position is based at Conway Health and Resource Center in Southwest, DC and offers a hybrid work arrangement. The ideal candidate will have a social work background or related field, at least one year in social services, strong

Qualifications

• Bachelor’s degree in social work or related field required.
• Minimum one year work experience in social service or related field.
• Valid driver’s license and regular access to a car with proof of auto insurance.

Responsibilities

• Develop case management plans updated monthly and signed by clients and case manager.
• Meet with families weekly to review progress and follow up on tasks and services.
• Coordinate with employment supports to address clients’ employment and educational needs.
• Network with community resources for housing, employment, substance use treatment, mental health, training, etc.
