Job summary

Community of Hope in Washington, DC is seeking a Housing Stability Case Manager to lead case management in homelessness prevention. You will develop plans with families, coordinate with employment and community resources, and ensure ongoing engagement and progress toward housing stability.

The role supports adults and children, requires a Bachelor’s degree or equivalent experience, and offers a hybrid work arrangement at Conway Health and Resource Center in Southwest DC.

Qualifications

• Bachelor’s degree in social work or related field or 4 years social service experience.
• Minimum one year work experience in social service or related field.
• Strong interpersonal, conflict-resolution, and organizational skills.
• Ability to work independently and as a team.
• A valid driver’s license and regular access to a car, as well as proof of auto insurance, required for staff working in scattered site housing programs.

Responsibilities

• Develop case management plans with monthly updates and client approval.
• Meet with families weekly to review progress and follow-up.
• Coordinate with employment supports to address needs.
• Network with housing, employment, substance abuse, mental health services, and refer as appropriate.
• Complete required assessments with families.

• Obtain signed release of information and coordinate with treatment and vocational programs.

This position is located at Conway Health and Resource Center in Southwest, DC.

What You’ll Do

• Works with adults and children to develop case management plans that are updated at least monthly and signed by clients and case manager, which incorporate goals for both adults and children related to core goal areas: finding stable housing; increasing income; and family stabilization and support.
• Meets weekly and as needed with families on caseload to develop case plans, review progress, and to follow-up on tasks and services.
• Coordinates with available employment supports to ensure that clients’ employment and educational needs are being effectively addressed.
• Networks with community resources for housing, employment, substance abuse treatment, mental health treatment, training, etc., and makes referrals as appropriate, including to COH Therapist and other community resources, as appropriate.
• Completes required assessments with families.
• Maintains regular communication in accordance with program contract guidelines with all families on caseload and documents progressive engagement efforts with difficult to engage families.
• Obtains signed release of information forms and communicates regularly with staff at treatment programs, substance abuse programs, and vocational programs.
